The tubes are for urethral catheters, so that one can pee wearing the cages, but my understanding is that those aren't really effective for long term wear without a lot of cleaning and constant changing of the tubes, much like you would with a medical catheter

Of course you can wear permanently flat cages or inverted cages!! I only wear flat chastity-cage, as I love the "no-cock-look" - and I wear them permanently since years!!!
